What Is a Nipple-Sparing Mastectomy?

A nipple-sparing mastectomy is a type of mastectomy where the surgeon removes the breast tissue but keeps the skin of the breast, along with the nipple and areola. The breast is then reconstructed, usually with an implant or your own tissue.

For many women, keeping their own nipple feels very important, and when it heals beautifully, it is a wonderful result. Your surgeon is the one who decides whether you are a candidate, based on your diagnosis and your body.

What I see in my studio is that every body heals in its own way. Some women come to me after a nipple-sparing mastectomy because the nipple and areola lost some of their color during healing, or healed lighter, uneven, or in a slightly different position than they hoped. Some have been through several surgeries along the way. Many of these women end up in my chair for a 3D areola tattoo to bring back color, shape, and balance. What Is Nipple Reconstruction Surgery?

When the nipple is not kept during mastectomy, some women choose to have a nipple rebuilt surgically after their breast reconstruction. There are a few common ways this is done:

Skin grafts: Skin from another area of the body is sometimes used to help create the nipple or areola.

Here is the part many women do not realize until after surgery: a surgically reconstructed nipple has no color. It is made from breast skin, so it is the same tone as the rest of the breast. To have a natural-looking nipple and areola, women still need tattooing afterward. That is why so many reconstruction clients come to me to complete the look.

What Is a 3D Areola Tattoo?

A 3D areola tattoo is a non-surgical option that creates the appearance of a natural nipple and areola using pigment. I use shading, highlights, and layered color to create the illusion of depth, so the nipple looks raised even when the skin underneath is flat.

This is where my work really shines. Because I am building the nipple and areola with color and light, I can control every detail: the size, the shape, the placement, the texture of the areola, and the exact tones that match your skin. I can make it look truly three-dimensional.

A lot of women tell me the same thing when they come in: they are tired of surgery. After a mastectomy, reconstruction, and sometimes revisions, the idea of another operation, another recovery, and more time off from life is just too much.

With a 3D areola tattoo, there is no surgery, no incision, and no hospital stay. The appointment takes place in my private studio, and I numb the area before I start so you are comfortable. Most clients go right back to their normal day.

Before and After: Adding Color and Dimension

In the first photo, you can see a reconstructed breast with a nipple but no color. The nipple blends into the breast skin, so the area does not look complete.

In the second photo, I added a 3D areola tattoo. The custom color gives the nipple and areola natural depth, warmth, and definition, and the whole breast looks balanced and finished.
